Facilitated Learning Analysis released for serious injury on Pyramid Butte fire

On September 4, 2010 a crewmember on the Warm Springs Hot Shot Crew was seriously injured by a large falling rock while working on the Pyramid Butte fire in Oregon.  On September 8 Wildfire Today posted an excerpt from the preliminary accident report HERE, and now the Wildland Fire Lessons Learned Center has released a Facilitated Learning Analysis for the incident.
